First Bronchial Artery Embolization for Hemoptysis in Manipur

Imphal, July 29: A revolutionary medical milestone was accomplished by Shija Hospitals with the successful treatment of acute bleeding in lungs known as hemoptysis (blood in sputum during cough) with an advanced treatment procedure known as Bronchial artery embolization. It is performed for various causes of Hemoptysis such as lung diseases (Tuberculosis, bronchiectasis, pulmonary AVM (Arteriovenous Malformation), aneurysm) etc. This highly complex, advanced and sophisticated treatment was performed for the first time in Manipur by Dr. Thangjam Gautam Singh, Consultant Interventional Radiologist by using a non-open surgical technique of blocking the bleeding artery in lungs under the guidance of the newly installed state-of-the-art DSA Cath lab at Shija hospitals. This technique involves entry inside the body by using only a ‘pin-sized’ hole in the artery either in leg (femoral) or hand (radial).

The patient was a 40 years old female with pulmonary tuberculosis with on and off hemoptysis since 2002. She developed pulmonary Aspergillosis (fungal ball) within the chronic TB cavity. She had completed Anti-tuberculosis drugs regimen and was on antifungal medications to which she did not respond and had persistent hemoptysis leading to low hemoglobin levels. Bronchial and systemic arteries angiography was performed which revealed multiple actively bleeding bronchial arteries. All bleeding arteries were immediately embolized (occluded or blocked) by embolic agents (PVA) with on-spot complete cessation of bleeding. Subsequently no hemoptysis occurred after the procedure and patient was discharged after 2 days.

Bronchial artery embolization offers unique advantages such as immediate, complete cessation of bleeding without open surgery, no pain, faster recovery and no scar.
